If your agency is new to our program, the following is needed to get you set up as producer.
- Current W9 (signed and dated)- http://www.irs.gov/pub/irs-
pdf/fw9.pdf - E&O Policy Dec Page or current certificate with minimum limits of $1,000,000 per occurrence
- Agency, Corporate and/or Individual Agent Licenses
- Name and email address of person responsible for signing the Unified Producer's Agreement
